Representing the Department of Education do hereby agree to the following:

A. Responsibilities of the First Party (MERLY V. CATULAY)

1. Commit to render voluntary services in school to check the health


precautionary measure compliance of parents/guardians/significant others on
the scheduled distribution and retrieval of learning modules.

2. Impose health safety protocols set by the school based on IATF protocols.

3. Report to school authority the identified problems that may arise during the
healthy safety checking among parents/guardians/significant others.

B. Responsibilities of the Second Party - Department of Education


(MANDUS ELEMENTARY SCHOOL)

1. Provide the First Party with the school rules and regulations / guidelines as his
reference to any actions that she will undertake.
2. Do necessary acts / measures to any reported incidents happened inside the
school premises.
3. Monitor/ Supervise performance of the first party and terminate contract if
unsatisfactory performance was found out.
