Template:SMW site new

This template, created to replace Template:SMW site (now deprecated), needs to be renamed and actual usage should be updated by an admin with access to something like ReplaceText.
Maybe rename to 'SMW site', but there is a good case to be made for simply 'Website' or 'Site'.

To use this template, insert the following at the beginning of your user page. All fields are optional and can be omitted if not applicable.

The order of the fields is not relevant. The template should be given as the first thing on a page.
